File a Claim Form in Person
You may choose to file your claim in person at one of our 35 Claims Site Offices located throughout the Gulf Coast Region (please click here to find a Site Office near you). Each Claims Site Office is staffed with claim evaluators who will assist you with the process. Before visiting one of our Claims Site Offices, you should be sure to gather certain information relative to your claim for relief and take it with you to the Claims Site Office.
If you are Filing as an Individual
Please bring with you a valid form of photo identification. Also bring with you documents necessary to support your claim. For a complete list of those documents, please see Exhibit A to the Claim Form, which provides a list of supporting documentation needed for each type of claim.
If you are Filing as a Business
